Dentures are an artificial set of teeth that replace all of a patient's upper or lower teeth. Dentures come out and in the wearer's mouth. They are removable. Dentures are made of plastic teeth with a gum colored plastic base. Good dentures fit the gums underneath the dentures very well.

A patient with dentures still has to see the dentist routinely because dentures must be relined regularly. When dentures wear down a dentures patient's face starts looking like there's nothing in her mouth. This is called over-closed.

Natural looking denture teeth should show when a denture patient smiles. When too much gum colored denture base shows, the denture patient's smile looks too gummy.

What is a Denture?

A denture is a removable replacement for missing teeth and adjacent tissues. It is made of acrylic resin, sometimes in combination with various metals. Complete dentures replace all the teeth, while a partial denture fills in the spaces created by missing teeth and prevents other teeth from changing position. Complete dentures are either "conventional" or "immediate." A conventional denture is placed in the mouth about a month after all the teeth are removed to allow for proper healing, whereas an immediate denture is placed as soon as the teeth are removed. The drawback behind an immediate denture is that it may require more adjustments after the healing has taken place.

Who needs a denture?
Candidates for complete dentures have lost most or all of their teeth. A partial denture is suitable for those who have some natural teeth remaining. A denture improves chewing ability and speech, and provides support for facial muscles. It will greatly enhance the facial appearance and smile.

What happens when you get a denture?
A dentist can make a full conventional denture when all teeth have been lost or all extraction sites have healed (up to eight weeks or longer.) The denture process takes about one month and five appointments: the initial diagnosis is made; an impression and a wax bite are made to determine vertical dimensions and proper jaw position; a "try-in" is placed to assure proper color, shape and fit; and the patient's final denture is placed, following any minor adjustments. New denture wearers need time to get accustomed to their new "teeth" because even the best fitting dentures will feel awkward at first. While most patients can begin to speak normally within a few hours, many patients report discomfort with eating for several days to a few weeks. To get accustomed to chewing with a new denture, start with soft, easy-to-chew foods. In addition, denture wearers often notice a slight change in facial appearance, increased salivary flow, or minor speech difficulty.

How do you care for a denture?
A denture is fragile, so it is important to handle it with care. Remove and brush the denture daily, preferably with a brush designed specifically for cleaning dentures, using either a denture cleanser or toothpastes. Never use harsh, abrasive cleansers, including abrasives toothpastes, because they may scratch the surface of the denture. Don't sterilize your denture with boiling water because it will cause it to become warped. If you wear a partial denture be sure to remove it before brushing your natural teeth. When not in use, soak it in a cleanser solution or in water. Get in the habit of keeping the denture in the same safe and handy place to reduce the likelihood of misplacement.

Should a denture be worn at night?
While you may be advised to wear your denture almost constantly during the first two weeks- even while you sleep-under normal circumstances it is considered best to remove it at night. Research has shown that removing the denture for at least eight hours during either the day or night allows the gum tissue to rest and allows normal stimulation and cleansing by the tongue and saliva. This promotes better long-term health of the gums.

Continue seeing your dentist regularly
It is important to continue having regular dental checkups so that a dentist can examine oral tissues for signs of disease or cancer. As of aging, your mouth will continue to change as the bone under your denture shrinks or recedes. To maintain a proper fit over time, it may be necessary to adjust your denture or possibly remake your denture. Never attempt to adjust a denture yourself and do not use denture adhesives for a prolonged period because it can contribute to bone loss. When in doubt, consult your dentist.

Are there any alternatives to dentures?
Dentures are no longer the only way to restore a mouth that has little or no non-restorable teeth. Strategically placed support, or implants, can now be used to support permanently cemented bridges, eliminating the need for a denture. The cost tends to be greater, but the implants and bridges more closely resemble the "feel" of real teeth. Dental implants are becoming the alternative of choice to dentures, but not everyone is a candidate for implants. Call your dentist for advice.

CHICAGO (June 1, 2003) - One-in-five adults, including half of the over-55 generation, wear some type of dentures and many denture-wearers may think regularly using an over-the-counter cleanser would keep dentures pearly white and more importantly, germ-free.

Yet, traditional denture soaking methods often do not leave dentures completely bacteria-free. Combining the soaking cleaning program with a two-minute microwave treatment was overwhelmingly more effective to eliminate germs on the inside and outside of dentures worn from 12 days to 48 years, according to a recent study published in a 2003 issue of General Dentistry, the clinical, peer-reviewed journal of the Academy of General Dentistry (AGD).

Wearing unclean dentures may cause the patient to develop denture-related stomatitis, a fungal infection in the mouth characterized by white or red sores, bleeding, swelling and burning sensations, according to R. Thomas Glass, DDS, PhD, a co-author of the study.

Dentures are made of a combination of porcelain and acrylic, and are worn when a tooth is broken and cannot be repaired or the decay is too vast to be eliminated and permanent bridges or implants are not an option. In addition to full dentures, partial dentures are made for two to up to 16 teeth and are easily removable.

"The research has shown toothbrushing of dentures is not adequate to combat stains and odors caused by bacteria," says Gordon R. Isbell, DMD, MAGD, and AGD spokesperson. According to Dr. Glass, the combination of denture cleaners and microwave was especially effective to remove microorganisms (bacteria, yeasts and fungi) found in the depths of the dentures.

How to Microwave Dentures

Place dentures that have no metal compartments in a microwave container at least twice as tall as the dentures, with vents in the cover
Fill the container with water
Drop one tablet of the cleanser in the water
Place towel over the cover of the microwave container to catch any excess fluid
Microwave the container for two minutes using a standard microwave
Allow denture to completely cool, then rinse and wear

Caution
Only use this method for complete dentures. DO NOT use this method for dentures that contain metal, since no testing has been conducted on such appliances.

Lost Your Denture? Check Your Stomach

Swallowing dental foreign objects is common, with X-rays often revealing dentures, toothbrushes and toothpicks lodged in patients' throats and stomachs, according to a recent report in General Dentistry, the peer-reviewed journal of the Academy of General Dentistry.

"Any foreign dental object that is swallowed and becomes lodged in the throat or stomach can cause a potentially life threatening situation," says Charles H. Perle, DMD, FAGD, spokesperson for the Academy of General Dentistry, an organization of general dentists dedicated to continuing education. "Accidentally swallowing a denture may happen during sleep, due to a poor fitting appliance or even possibly during an intoxicated episode."

Denture wearers must remember that the bone underneath their denture will change, changing the shape of the mouth, but not of the denture, in turn causing a loose-fitting appliance. In addition to increasing the chance of swallowing the appliance, a loose fitting denture cause loss of palate sensitivity making the patient more prone to swallowing chicken or fish bones.

Dr. Perle has also treated non-denture wearing patients that have ingested pennies, toys, pen caps, bobbie pins and sewing needles.

Additional case reports reveal:
Ingested toothpicks have caused liver abscesses. While inducing purging, bulimics have accidentally ingested toothbrushes. Accidental ingestion of partial dentures and orthodontic appliances has required surgical removal procedures.

More than 2700 Americans choke to death each year. Protect yourself if you wear dentures:

Visit the dentist twice a year to make sure the appliance is in good working order.
Consider a fixed bridge or implants instead of a partial denture.
If you have severe bone loss and still need a denture, ask about getting implants to stabilize the denture.
Do not wear faulty appliances overnight. There is a danger of swallowing during sleep.
Remove the appliance before playing sports.
